PP405 Hair Follicle Research: Phase 2/3 Trials & Mechanism (2026)

PP405 research update: mitochondrial pyruvate carrier mechanism, Phase 2a findings, the 2026 AWAKEN Phase 2/3 trial, and what human evidence does not prove.

2026 Evidence Brief / Follicle Metabolism

PP405 (PP-405) is an investigational small molecule being studied for its ability to influence the metabolic state of hair-follicle stem cells. Here is what the current clinical program actually shows, why mitochondrial pyruvate transport matters, and what remains unknown.

Compound classSmall molecule, not a peptide
Investigational targetMitochondrial pyruvate carrier (MPC)
Clinical statusPhase 2/3 AWAKEN initiated in 2026

What is PP405, and how does it work?

PP405 is a topical drug candidate being developed by Pelage Pharmaceuticals. Its proposed approach is to inhibit mitochondrial pyruvate transport, altering the way cells process carbon from glucose. Foundational preclinical work found that reducing mitochondrial pyruvate carrier activity could increase lactate-associated metabolism and activate otherwise resting hair-follicle stem cells in mouse models.

This mechanism differs from the androgen / dihydrotestosterone (DHT) pathway associated with finasteride. PP405 development focuses on a metabolic switch within the follicle, not on 5Ξ±-reductase inhibition. Mechanistic plausibility should not be confused with demonstrated long-term human outcomes.

What did the PP405 Phase 2a study report?

Pelage’s June 17, 2025 company communication described a randomized Phase 2a study enrolling 78 adults with androgenetic alopecia. Participants used the investigational topical formulation or a vehicle comparator for four weeks, with follow-up thereafter. The company reported that its primary safety endpoint was met and that plasma exposure was not detected using the study’s assays.

Early signal, not a cure claim.

Among a subgroup of men with more advanced hair loss, Pelage reported that 31% of participants receiving PP405 achieved more than a 20% increase in hair density at week eight, compared with 0% of the corresponding placebo subgroup. These are developer-reported exploratory findings; they are not proof that every patient would benefit or that a retail research solution reproduces the trial result.

When reading such results, distinguish the study’s primary safety endpoint from exploratory efficacy measures, examine subgroup denominators, and wait for larger controlled datasets before drawing firm conclusions about durability and clinical effectiveness.

Latest update: the 2026 AWAKEN Phase 2/3 trial

On October 14, 2026, Pelage announced dosing of the first participant in the AWAKEN Phase 2/3 program, ClinicalTrials.gov identifier NCT07819435. The randomized, blinded, vehicle-controlled study is designed to enroll about 600 men with androgenetic alopecia and assess efficacy and longer-term safety over a 12-month study period.

This is a meaningful development milestone, not FDA approval. PP405 remains an investigational drug. The study formulation, clinical oversight and drug supply are not interchangeable with products sold for laboratory research.

PP405 research FAQs

Is PP405 the same as PP-405?

These spellings refer to the same development code. The molecular identity should nevertheless be checked against a lot-specific analytical record rather than inferred from a marketing name.

Is PP405 FDA approved for hair loss?

No. As of October 29, 2026, the compound remains investigational, with a Phase 2/3 clinical program in progress.

How does PP405 differ from KY19382?

PP405 targets the follicle’s metabolic state via the mitochondrial pyruvate carrier. KY19382 is a preclinical Wnt/Ξ²-catenin pathway compound studied in cells, cultured follicles and mice. Compare the KY19382 research.

Does the clinical result validate a retail PP405 solution?

No. Clinical findings belong to the specific investigational product, study population, protocol and analytical procedures. Check the COA portal for the information reported on each Disguised Alpha lot.
