What are exercise mimetics?

Exercise mimetics are compounds studied for their ability to switch on parts of the gene program that exercise sets off in skeletal muscle, in animals that are not exercising. A 2021 review calls them compounds that mimic or potentiate the effects of exercise, or "exercise pills", and notes that most target skeletal muscle (Hawley et al., 2021). The landmark study came in 2008, when drugs acting on AMP-activated protein kinase (AMPK) and peroxisome proliferator-activated receptor delta (PPARδ) reprogrammed mouse muscle (Narkar et al., 2008).

Related research covers estrogen-related receptor (ERR) agonists, a REV-ERB agonist, a mitochondrial uncoupler and a peptide encoded in mitochondrial DNA, almost all of it in mice and cultured cells. Disguised Alpha supplies eight of the exercise mimetic compounds covered here, as capsules, lyophilized powders and liquid research formulas, for laboratory research use only. Nothing on this page is guidance for use in people.

The landmark 2008 mouse study

Salk Institute researchers asked whether drugs could reproduce what training does to mouse muscle. In sedentary mice, four weeks of the PPARδ agonist GW1516 (GW501516, known as Cardarine) switched on oxidative genes in muscle but changed neither treadmill running nor fiber type. Combined with four weeks of training, it raised running time by 68 percent and distance by 70 percent over trained mice given vehicle.

Because training activates AMPK and the coactivator PGC-1α, the team then gave sedentary mice the AMPK activator AICAR for four weeks. With no training, AICAR switched on 32 genes linked to oxidative metabolism in the quadriceps, and the mice ran about 23 percent longer and 44 percent farther than vehicle controls (Narkar et al., 2008).

The label drew objections. In 2009, two physiologists argued that the "exercise pill" idea lacked a scientific basis and underrated how cells, tissues and organs work together during exercise. Drugs aimed at one or a few molecules, they wrote, should not redefine the word exercise (Booth and Laye, 2009). The 2021 review adds that muscle-targeted compounds miss many effects of exercise on other organs, including the cardiovascular system (Hawley et al., 2021).

The pathway families

Several families converge on one muscle program: loss of REV-ERBα in muscle, for example, switches off an LKB1, AMPK, SIRT1 and PGC-1α chain (Woldt et al., 2013).

AMPK activators: AICAR and ATX-304 (O304)

AMPK is a master regulator of energy balance that physical activity and caloric restriction switch on (Steneberg et al., 2018). In AMPK activators research, AICAR is the long-standing tool compound and one of the two drugs in the 2008 study. O304, later coded ATX-304, was reported in 2018 as a pan-AMPK activator. In diet-induced obese mice it raised glucose uptake in skeletal muscle. Like exercise, it also activated AMPK in the heart and raised left ventricular stroke volume without raising heart weight (Steneberg et al., 2018).

PPAR delta agonists: GW501516 (Cardarine)

GW501516 is a synthetic agonist of PPARδ, a nuclear receptor. In the 2008 study it changed treadmill running only together with training (Narkar et al., 2008). GlaxoSmithKline announced it would discontinue further research after preclinical studies in rodents linked the compound to widespread tumor development (Mackenzie and Lione, 2013).

ERR agonists: SLU-PP-332 and SLU-PP-915

Estrogen-related receptors alpha, beta and gamma are orphan nuclear receptors that genetic studies tie to muscle exercise capacity. SLU-PP-332, from Saint Louis University, activates all three, most potently ERRα. It raised mitochondrial respiration in a muscle cell line. In mice it increased type IIa oxidative fibers, lengthened treadmill running and induced an acute aerobic exercise gene program that depended on ERRα (Billon et al., 2023).

SLU-PP-915 is a later pan-ERR agonist with a different chemical structure. A 2026 systematic review found that both switch on acute aerobic exercise genes such as Ddit4 and, drawing only on animal and cell studies, called for clinical trials (de Souza-Lima et al., 2026).

REV-ERB agonists: SR-9009

REV-ERBα and REV-ERBβ are nuclear receptors that link circadian rhythms and metabolism (Dierickx et al., 2019). REV-ERBα is highly expressed in oxidative muscle, and its loss there reduced mitochondrial content. Mice given the synthetic agonist SR9009 ran longer, in time and distance, than vehicle controls, and SR9009 raised mitochondrial content in C2C12 muscle cells (Woldt et al., 2013).

Mitochondrial uncouplers: BAM15

Uncouplers carry protons into the mitochondrial matrix by a route that bypasses ATP synthase, uncoupling nutrient oxidation from ATP production and speeding respiration. BAM15 does this without depolarizing the plasma membrane. In cultured cells it supported higher maximal respiration than FCCP, an uncoupler of equal potency, with less cytotoxicity (Kenwood et al., 2014). The World Anti-Doping Agency (WADA) lists BAM15 among its examples of AMPK activators, beside AICAR and MOTS-c.

A mitochondria-derived peptide: MOTS-c

MOTS-c is a 16-amino-acid peptide encoded in the mitochondrial 12S rRNA gene. Its apparent main target organ is skeletal muscle, and in cells it inhibits the folate cycle and linked purine synthesis, leading to AMPK activation (Lee et al., 2015). In mice aged 2, 12 and 22 months it raised physical capacity, and in people, exercise raised natural MOTS-c in muscle and blood (Reynolds et al., 2021).

What the evidence does and does not show

It shows that single compounds can switch on parts of the muscle exercise program in sedentary rodents and cultured cells: oxidative genes, mitochondrial content and fiber type, sometimes with longer treadmill runs. Several key papers come from groups that disclose commercial interests in the compounds (Steneberg et al., 2018; Billon et al., 2023).

It does not show a working exercise pill for people. None of these compounds is an approved medicine. As acadesine, AICAR was tested in heart surgery, and one large trial stopped for futility with the primary outcome in 5.1 percent on acadesine and 5.0 percent on placebo (Newman et al., 2012). O304 has a phase I study and one exploratory 28-day phase IIa trial in 65 people with type 2 diabetes, whose fasting glucose finding came from a post hoc subgroup (Steneberg et al., 2018).

GW501516 reached clinical trials before development stopped (Mackenzie and Lione, 2013). MOTS-c has been measured in people (Reynolds et al., 2021), but PubMed indexes no trial giving it, SLU-PP-332, SLU-PP-915, SR-9009 or BAM15 to people (searched October 2026). A 2026 case report describes acute liver failure needing an emergency transplant in a 17-year-old after SR9009 use (Shams Bin Shaheen et al., 2026).

Two cautions apply to every pathway label. Tool compounds are not perfectly selective: SR9009 still changed cell viability, metabolism and gene transcription in cells lacking both REV-ERBs (Dierickx et al., 2019). And exercise reaches far beyond muscle, so copying part of the muscle response leaves the rest out (Hawley et al., 2021).

Anti-doping status

The 2026 WADA Prohibited List, in force from 1 January 2026, lists metabolic modulators as class S4.4, prohibited at all times as non-Specified Substances. Subclass S4.4.1 covers AMPK activators, naming BAM15, AICAR and MOTS-c as examples; PPARδ agonists, naming GW1516 (GW501516); and Rev-erbα agonists, naming SR9009 and SR9011.

ATX-304 (O304) is not named, although its papers describe it as an AMPK activator. SLU-PP-332 and SLU-PP-915 are not named either; class S0 covers any pharmacological substance not addressed by another section of the List and with no current approval by any governmental regulatory health authority for human therapeutic use. The list is revised every year.

Frequently asked questions

Is there an "exercise in a pill"?

Not for people. Exercise in a pill research is preclinical, and the clearest findings are in sedentary mice. None of these compounds is an approved medicine, and critics say single-target drugs should not redefine exercise (Booth and Laye, 2009).

What are ERR agonists?

Synthetic compounds that activate the estrogen-related receptors ERRα, ERRβ and ERRγ. SLU-PP-332 and SLU-PP-915 are the focus of a 2026 systematic review that covered only animal and cell studies (de Souza-Lima et al., 2026).

What is a PPAR delta agonist?

A compound that activates the nuclear receptor PPARδ. In PPAR delta agonist research, GW501516 is the compound from the 2008 mouse study, and its development stopped after preclinical studies in rodents linked it to widespread tumor development (Mackenzie and Lione, 2013).

Have exercise mimetics been tested in people?

Only a few, and for other purposes: acadesine (AICAR) in heart surgery (Newman et al., 2012), O304 in early trials in type 2 diabetes (Steneberg et al., 2018) and GW501516 in trials before its development stopped (Mackenzie and Lione, 2013). PubMed lists no trial of the ERR agonists, SR-9009 or BAM15.

Are exercise mimetics banned in sport?

Several are. The 2026 WADA Prohibited List names AICAR, BAM15, MOTS-c, GW1516 and SR9009 in class S4.4.1, prohibited at all times.
