Research Overview

Tesamorelin vs ipamorelin compares two routes to the same pituitary output. Tesamorelin is a stabilized analog of human growth hormone releasing hormone (GHRH), the hypothalamic peptide that acts through the GHRH receptor on pituitary somatotrophs (Ferdinandi et al., 2007; Cunha and Mayo, 2002). Ipamorelin is a synthetic pentapeptide acting at the growth hormone secretagogue receptor (GHS-R1a), whose natural ligand was later identified as ghrelin (Raun et al., 1998; Kojima et al., 1999).

The difference between tesamorelin and ipamorelin is also why studies pair them. The GHRH receptor raises cyclic AMP; the secretagogue receptor works through phospholipase C and calcium. In cells carrying both cloned receptors, activating the secretagogue receptor alone left cyclic AMP unchanged, yet activating both gave about twice the cyclic AMP response of GHRH alone (Cunha and Mayo, 2002). Disguised Alpha supplies tesamorelin, ipamorelin and a two-component Tesamorelin & Ipamorelin blend as lyophilized powders for laboratory research use only. Structure and Class

Tesamorelin keeps the full 44-residue human GHRH sequence, YADAIFTNSYRKVLGQLSARKLLQDIMSRQQGESNQERGARARL, with an amidated C-terminus, and adds a trans-3-hexenoyl group to the N-terminal tyrosine. That change made the peptide resistant to dipeptidyl aminopeptidase-IV and slowed its breakdown in rat, dog and human plasma (Ferdinandi et al., 2007). It was developed as TH9507. The product page lists C221H366N72O67S, 5135.86 g/mol, matching PubChem CID 16137828 (5136 g/mol, rounded).

Ipamorelin is far smaller: the pentapeptide Aib-His-D-2-Nal-D-Phe-Lys-NH2, with two non-natural residues and two D-amino acids, so it is not a fragment of any human protein. It came from a series of compounds lacking the central Ala-Trp dipeptide of GHRP-1 (Raun et al., 1998). The product page lists C38H49N9O5, 711.86 g/mol, matching PubChem CID 9831659.

Per molecule, tesamorelin weighs about 7.2 times as much, so the same milligram amount of ipamorelin holds about 7.2 times as many molecules. In the 8MG/2MG blend that is about 1.6 µmol of tesamorelin beside about 2.8 µmol of ipamorelin, so ipamorelin is the larger component by moles. Salt form and net peptide content are certificate values for each lot.

Research Context: Tesamorelin

In non-clinical work, daily repeat exposure to TH9507 raised plasma growth hormone and IGF-1 in pigs, rats and dogs, and the apparent elimination half-life in dogs was 21 to 45 minutes. In studies of up to four months, reversible adverse findings in dogs, including liver and kidney changes and anemia, were attributed to sustained supraphysiological growth hormone and IGF-1 (Ferdinandi et al., 2007).

In 13 healthy men over two weeks of once-daily exposure, tesamorelin raised mean overnight growth hormone, pulse area, basal secretion and IGF-1, while fasting glucose and insulin-stimulated glucose uptake did not change significantly (Stanley et al., 2011). Pulsatile secretion was amplified, not replaced.

In a randomized, placebo-controlled trial of 412 adults with HIV and abdominal fat accumulation, visceral adipose tissue measured by CT fell 15.2% over 26 weeks with tesamorelin and rose 5.0% with placebo, while IGF-1 rose 81% (Falutz et al., 2007). In November 2010 the FDA approved a pharmaceutical tesamorelin product, Egrifta, for one indication: reducing excess abdominal fat in HIV-associated lipodystrophy (Grunfeld et al., 2011). The research-grade material sold here is not that product and is not for any clinical use. The tesamorelin research guide covers the later trials.

Research Context: Ipamorelin

In the founding paper, ipamorelin released growth hormone from primary rat pituitary cells with potency similar to GHRP-6, acted through a GHRP-like receptor, not the GHRH receptor, in antagonist profiling, and released growth hormone in rats and conscious swine. In swine it did not raise ACTH or cortisol beyond the levels seen with GHRH, while GHRP-6 and GHRP-2 did (Raun et al., 1998). The receptor's natural ligand, ghrelin, an acylated 28-residue stomach peptide, was identified the next year (Kojima et al., 1999).

In healthy male volunteers at five escalating infusion rates, ipamorelin showed proportional kinetics, a terminal half-life of about two hours and one episode of growth hormone release peaking near 40 minutes (Gobburu et al., 1999).

Later work turned to postoperative ileus, since ghrelin receptor stimulation promotes gut motility. In rats after laparotomy and intestinal manipulation, a single exposure shortened the time to first bowel movement, and repeated exposure over two days increased fecal pellet output and food intake (Venkova et al., 2009).

A double-blind, placebo-controlled phase 2 trial in 114 adults after bowel resection found ipamorelin well tolerated, with no significant difference from placebo in its key or secondary efficacy analyses; median time to a tolerated solid meal was 25.3 against 32.6 hours (Beck et al., 2014). Clinical development stops there in the published record: PubMed lists no later ipamorelin trial, and no ipamorelin drug has been approved.

FDA places ipamorelin acetate in category 2 under its 503B compounding interim policy, added September 29, 2023. The agency's list of bulk drug substances that may present significant safety risks cites possible immunogenicity for certain routes and a published study reporting serious adverse events, including death, with intravenous ipamorelin for gastric motility. More is in the ipamorelin research guide.

Choosing Between Them for a Research Question

Choose by receptor first. GHRH receptor questions, cyclic AMP signaling in somatotrophs, or a stabilized analog that keeps pulsatile release point to tesamorelin. Ghrelin receptor questions, calcium-coupled signaling, selectivity against ACTH and cortisol, or gut motility models point to ipamorelin. The two half-lives come from different species, so neither is a sampling schedule for another model.

Combined designs rest on GHRH plus secretagogue work in animals. In mice lacking the GHRH gene, five weeks of a GHRH analog plus the secretagogue GHRP-2 produced greater body length and weight than the analog alone, and GHRP-2 released growth hormone only when given with the analog, suggesting the secretagogue response needs GHRH (Fintini et al., 2005). In preweaning calves, ghrelin and GHRH together released growth hormone synergistically, an effect lost after weaning (ThidarMyint et al., 2008). Neither study used tesamorelin or ipamorelin, and PubMed holds no study of the two together.

A synergy claim needs four arms: vehicle, each compound alone and the combination, which separate vials allow at any ratio. The Tesamorelin & Ipamorelin blend fixes 8MG to 2MG per vial and suits work that needs that exact mixture; the blend research guide covers it. GHRH-deficient and intact models may answer the same question differently.

Handling and Storage

The tesamorelin, ipamorelin and blend product pages all list -20°C for the lyophilized powder as shipped and 2 to 8°C after reconstitution, protected from light, with freeze/thaw cycles avoided. Let the sealed vial reach room temperature before opening, add diluent slowly down the vial wall, and swirl rather than shake.

In solution the two differ. Tesamorelin can cloud or gel in near-neutral, unbuffered diluents and at high concentration, and its methionine at position 27 oxidizes readily; the tesamorelin gelling note covers diluent choice. Ipamorelin, a C-terminally amidated pentapeptide with no methionine or cysteine, is sturdier but no more tolerant of freeze/thaw. No published data describe the two mixed in solution. Aliquot single-use volumes, record the lot number, keep its certificate from the COA portal, and use its measured content for molar calculations.

Questions Researchers Ask

What is the difference between tesamorelin and ipamorelin?

Tesamorelin is a 44-residue GHRH analog (Ferdinandi et al., 2007); ipamorelin is a five-residue ghrelin receptor agonist (Raun et al., 1998; Venkova et al., 2009). Tesamorelin has a randomized trial program and one approved drug product; ipamorelin has volunteer pharmacokinetics and one phase 2 trial that found no significant difference from placebo (Beck et al., 2014).

Tesamorelin or ipamorelin: which fits a growth hormone release study?

GHRH receptor and cyclic AMP questions point to tesamorelin; ghrelin receptor, calcium signaling and ACTH or cortisol selectivity questions point to ipamorelin; questions about their interaction need both.

Has tesamorelin been studied with ipamorelin?

Not directly. A PubMed search in October 2026 returned only reviews that mention both. The pairing rests on GHRH and secretagogue work in engineered cells, knockout mice and calves (Cunha and Mayo, 2002; Fintini et al., 2005; ThidarMyint et al., 2008).

Ipamorelin vs tesamorelin: are they approved or restricted?

A pharmaceutical tesamorelin product received FDA approval in November 2010 for one HIV-related indication (Grunfeld et al., 2011); ipamorelin has no approved product. The WADA Prohibited List for 2026 names both in class S2.2.4, growth hormone releasing factors, prohibited at all times.
