What Are
Racetams?

The shared chemistry of the racetam family, how piracetam, aniracetam, oxiracetam and their relatives differ, and what the research shows.

What are racetams?

Racetams are synthetic compounds built on a 2-oxopyrrolidine ring, also called 2-pyrrolidinone: a five-membered lactam. The family is named after piracetam, 2-(2-oxopyrrolidin-1-yl)acetamide (PubChem CID 4843), the first racetam, and most members share its acetamide side chain. A 2001 Lancet review traces piracetam to Corneliu Giurgea's research in the late 1960s and credits him with the word "nootropic" (Shorvon, 2001), a concept he set out in the early 1970s (Giurgea, 1973). Disguised Alpha supplies several racetam compounds and related materials for laboratory research use only. Nothing on this page is guidance for use in people.

The shared core and how racetams differ

Every racetam keeps the 2-oxopyrrolidine ring; the members differ in what is attached to it. Structures are from PubChem.

  • Ring carbon 4: a hydroxyl group in oxiracetam (CID 4626) and a phenyl group in phenylpiracetam (CID 132441), each creating a stereocenter.
  • Amide nitrogen: a 2,6-dimethylphenyl group in nefiracetam (CID 71157) and a 2-(diisopropylamino)ethyl group in pramiracetam (CID 51712).
  • Ring nitrogen: aniracetam carries a 4-methoxybenzoyl group instead of the acetamide (CID 2196).
  • Ring carbon 5: fasoracetam carries a piperidine carbonyl group, in the R form, and leaves the ring nitrogen free (CID 198695).
  • Side chain: levetiracetam, the S-enantiomer of etiracetam, adds an ethyl group beside the amide (CIDs 5284583 and 59708); brivaracetam also adds a 4-propyl group (CID 9837243).

At the bench, enantiomers co-elute on a standard, non-chiral HPLC column, and nefiracetam and sunifiram share the formula C₁₄H₁₈N₂O₂ and a weight of 246.30 g/mol (CIDs 71157 and 4223812), so mass alone cannot tell them apart. Disguised Alpha's racetam capsule pages list storage at 15 to 25°C, sealed, away from moisture and light, and our COA explainer covers identity tests.

From piracetam to a family of racetam compounds

UCB, described in a 2001 Cochrane review as marketing most of the world's piracetam (Flicker and Grimley Evans, 2001), also holds the original US approvals for levetiracetam and brivaracetam. A 2010 review sorted the piracetam-like drugs into three subgroups: piracetam, oxiracetam, aniracetam, pramiracetam and phenylpiracetam, which have been used clinically and in some cases sold as dietary supplements; levetiracetam, seletracetam and brivaracetam, which bind the synaptic vesicle protein SV2A; and derivatives of unknown clinical efficacy, such as nefiracetam (Malykh and Sadaie, 2010). Fasoracetam came later, from Nippon Shinyaku (Hirouchi et al., 2000).

How racetam compounds differ in mechanism

Racetam research has not identified one target shared by the whole family; the 2010 review called the mode of action of piracetam and most derivatives an enigma (Malykh and Sadaie, 2010).

AMPA receptors: aniracetam, piracetam and oxiracetam

In cultured cerebellar granule cells, micromolar piracetam, aniracetam and oxiracetam each raised AMPA-stimulated calcium influx, increasing efficacy rather than potency, and raised AMPA binding site density in rat cortical membranes (Copani et al., 1992). Crystal structures of the receptor's ligand-binding domain show two of the first known AMPA receptor allosteric modulators binding differently: aniracetam at one symmetrical site in the center of the interface between two subunits, piracetam at several low-occupancy sites along it (Ahmed and Oswald, 2010).

Calcium channels and nicotinic receptors: nefiracetam

In NG108-15 neuroblastoma-glioma cells, 1 µM nefiracetam doubled L-type calcium channel currents and helped N-type currents recover from Go/Gi-mediated inhibition; aniracetam acted more weakly, and piracetam and oxiracetam not at all (Yoshii et al., 2000). In cultured rat cortical neurons, 1 nM nefiracetam potentiated α4β2-type nicotinic receptor currents to 200 to 300 percent of control through Gs proteins, yet it inhibited human α4β2 receptors in a human cell line (Zhao et al., 2001).

Metabotropic glutamate receptors: fasoracetam

In rat cortical membranes and cultured mouse cortical neurons, fasoracetam, coded NS-105, lowered forskolin-stimulated cAMP formation through group II and III metabotropic glutamate receptors and, after pertussis toxin pretreatment, raised it through group I receptors (Hirouchi et al., 2000).

SV2A: the antiseizure branch

In 2004, a team led from UCB showed that the synaptic vesicle protein SV2A is levetiracetam's binding site in the brain: membranes from mice lacking SV2A did not bind a tritiated levetiracetam derivative, and SV2A affinity across related compounds tracked seizure protection in audiogenic mice (Lynch et al., 2004). This branch appears here only as pharmacology history.

What the human evidence shows

The human record is mixed, and much of it is older or small. A 2001 Cochrane review of piracetam found many crossover trials with unavailable first-period data. Only global impression of change had enough pooled data; it favored piracetam (fixed-effects odds ratio 3.55, with heterogeneity), while cognitive measures were inconclusive, and the reviewers concluded that the published evidence did not support piracetam for dementia or cognitive impairment (Flicker and Grimley Evans, 2001).

A 2024 meta-analysis of 18 placebo-controlled studies in 886 adults with memory impairment found no clear memory difference (standardized mean difference 0.75, 95% CI -0.19 to 1.69, I² 96%) (Gouhie et al., 2024). The 2010 review reported no cognitive effect of nefiracetam after stroke (Malykh and Sadaie, 2010). Each guide covers its compound's trials as research history; PubMed indexes no study of sunifiram in people (searched October 2026).

Regulatory status

The US Food and Drug Administration (FDA) has not approved any racetam for memory or cognition: searched through openFDA in October 2026, Drugs@FDA lists no product containing piracetam, oxiracetam, aniracetam, pramiracetam, phenylpiracetam, nefiracetam or fasoracetam. Two members are FDA-approved antiseizure medicines from UCB: levetiracetam (1999) and brivaracetam (2016). In Europe, a European Medicines Agency list dated January 2023 shows piracetam medicines nationally authorised in more than 20 countries, almost all held by UCB companies.

In a 2019 warning letter, the FDA said one seller's aniracetam, oxiracetam, phenylpiracetam, piracetam, pramiracetam and Noopept products, sold with memory and disease claims, were unapproved new drugs and not dietary supplements, while noting that the letter did not address whether piracetam products can be lawfully marketed as dietary supplements.

Compounds often grouped with racetams

Noopept: a dipeptide derivative

Noopept (omberacetam, GVS-111) is N-phenylacetyl-L-prolylglycine ethyl ester (PubChem CID 180496). Its nitrogen ring is the pyrrolidine of proline, with no ring carbonyl, so it lacks the racetam core. Moscow researchers described it as a dipeptide piracetam analog; one hour after rats received it, it was undetectable in brain while cycloprolylglycine had risen 2.5-fold, and they proposed that it acts as a prodrug (Gudasheva et al., 1997).

Sunifiram: a piperazine

Sunifiram (DM-235) is 1-(4-benzoylpiperazin-1-yl)propan-1-one (PubChem CID 4223812), a piperazine. Its University of Florence developers describe it and unifiram as structurally related to ampakines, a class of AMPA receptor modulators. In mice it reversed amnesia caused by the AMPA antagonist NBQX, which abolished its effect in rat hippocampal slices (Galeotti et al., 2003). These are animal and tissue data only.

Phenylpiracetam: a phenyl derivative of piracetam

Phenylpiracetam (fonturacetam, carphedon) is piracetam with a phenyl group on ring carbon 4 (PubChem CID 132441). Unlike Noopept and sunifiram, it keeps the racetam core, and the 2010 review groups it with piracetam as a more potent relative (Malykh and Sadaie, 2010). In radioligand binding assays, the dopamine transporter was the only significant target found for its R-enantiomer (Zvejniece et al., 2020). The World Anti-Doping Agency's 2026 Prohibited List names fonturacetam [4-phenylpiracetam (carphedon)] among the non-specified stimulants, class S6.A, prohibited in competition.

Frequently asked questions

What are racetams?

Synthetic compounds that share a 2-oxopyrrolidine ring, named after piracetam, the first of them. The family dates to Corneliu Giurgea's piracetam research in the late 1960s (Shorvon, 2001).

How many types of racetams are there?

There is no official list. A 2010 review sorted them into three subgroups: piracetam and close relatives, the SV2A-binding antiseizure compounds, and other derivatives such as nefiracetam (Malykh and Sadaie, 2010).

Is Noopept a racetam?

Not by structure: it is a dipeptide derivative with no 2-oxopyrrolidine ring, though its nonproprietary name, omberacetam, has the "-racetam" ending. Its developers called it a dipeptide piracetam analog (Gudasheva et al., 1997).

Is phenylpiracetam a racetam?

Yes, by structure: it is piracetam with a phenyl group on ring carbon 4. As fonturacetam, it is on the WADA Prohibited List as a stimulant, prohibited in competition.

Is levetiracetam a racetam?

By structure, yes: it is the S-enantiomer of etiracetam, an ethyl analog of piracetam. It binds SV2A (Lynch et al., 2004) and is an FDA-approved antiseizure medicine, not supplied by Disguised Alpha.

Are racetams FDA approved?

Only levetiracetam (1999) and brivaracetam (2016), as antiseizure medicines. Drugs@FDA lists no product containing piracetam, aniracetam, oxiracetam, pramiracetam, phenylpiracetam, nefiracetam or fasoracetam (openFDA search, October 2026).

Where can researchers buy racetam compounds?

Disguised Alpha lists Piracetam, Aniracetam, Oxiracetam, Nefiracetam, Fasoracetam and Phenylpiracetam Aerosol Spray, plus the related Noopept and Sunifiram, as research capsules, with lot certificates on each product page and in the COA portal.
