Research Overview

Tesamorelin vs HGH compares a releasing-hormone analog with the hormone it releases. Tesamorelin is an analog of growth hormone-releasing hormone (GHRH) (Ferdinandi et al., 2007).

FDA labeling describes GHRH as a hypothalamic peptide that acts on pituitary somatotroph cells to stimulate the synthesis and pulsatile release of endogenous growth hormone (GH) (FDA prescribing information for Egrifta), and a 2026 review notes that the effects of GHRH analogs stay largely shaped by somatostatin tone and IGF-1 negative feedback (Dominikowski et al., 2026). Human growth hormone is the 191-residue hormone itself: it binds the GH receptor, activates JAK2/STAT signaling and promotes IGF-1 production (Dominikowski et al., 2026).

Disguised Alpha supplies tesamorelin (listed in 5MG and 10MG) and a Tesamorelin & Ipamorelin blend (8MG/2MG) as lyophilized powders for laboratory research use only. It does not sell growth hormone (somatropin). The HGH Fragment 176-191 it lists is a 16-residue piece of the hormone's C-terminal end, not the hormone. Nothing on this page is guidance for use in people.

Structure and Class

Tesamorelin keeps the full human GHRH sequence, YADAIFTNSYRKVLGQLSARKLLQDIMSRQQGESNQERGARARL, with an amidated C-terminus (PubChem CID 16137828), and adds a trans-3-hexenoyl group to tyrosine 1. Developed as TH9507, it resisted dipeptidyl aminopeptidase-IV and broke down more slowly than native GHRH in rat, dog and human plasma (Ferdinandi et al., 2007). FDA labeling states that it binds and stimulates human GRF receptors in vitro with potency similar to endogenous GRF (FDA prescribing information for Egrifta). Product page identity: C221H366N72O67S, 5135.86 g/mol.

GH is a protein about 4.3 times heavier. FDA labeling describes somatropin as human GH made by recombinant DNA technology in Escherichia coli, with 191 amino acid residues, a molecular weight of 22,124 daltons and a sequence identical to pituitary GH (FDA prescribing information for a somatropin product). The crystal structure of GH bound to its receptor's extracellular domain showed a four-helix bundle, with one hormone molecule binding two receptor molecules (de Vos et al., 1992).

Research Context: Tesamorelin

As TH9507, tesamorelin raised plasma GH and IGF-1 in pigs, rats and dogs on daily repeat exposure. Its apparent elimination half-life in dogs was 21 to 45 minutes, and reversible adverse findings in dogs were attributed to sustained supraphysiological GH and IGF-1 (Ferdinandi et al., 2007). Models: pigs, rats and dogs.

In 13 healthy men over two weeks of once-daily exposure, tesamorelin raised mean overnight GH, GH peak area and basal secretion, and IGF-I rose, while fasting glucose and insulin-stimulated glucose uptake did not change significantly; the authors concluded that it augments basal and pulsatile GH secretion (Stanley et al., 2011). Model: healthy men.

In a randomized, placebo-controlled 26-week trial of 412 adults with HIV and abdominal fat accumulation, the primary endpoint was visceral adipose tissue measured by CT; IGF-I rose 81.0% with tesamorelin and fell 5.0% with placebo, glycemic measures did not differ significantly, and more tesamorelin participants withdrew because of an adverse event (Falutz et al., 2007). In November 2010 the FDA approved Egrifta, a tesamorelin drug product, for an indication in HIV-infected patients with lipodystrophy (Grunfeld et al., 2011). That approval covers that drug product only, not the research material sold here.

Research Context: Human Growth Hormone

GH leaves the pituitary in a pulsatile, sleep-linked pattern under hypothalamic control (Dominikowski et al., 2026). In human subjects its most prominent metabolic effect is a marked increase in lipolysis and free fatty acid levels, and it antagonizes the effects of insulin on glucose metabolism (Møller and Jørgensen, 2009).

The hormone feeds back on its own axis. In six normal men, five days of biosynthetic methionyl human GH inhibited the GH response to GHRH(1-44) while somatomedin-C rose (Rosenthal et al., 1986). In 17 trained men randomized to recombinant GH or placebo for one week, total and 22 kDa GH rose while endogenous 20 kDa and non-22 kDa GH were suppressed, and exercise-stimulated endogenous isoforms stayed suppressed for up to four days after the last exposure (Wallace et al., 2001).

Older material differed in source. A 2000 review reported that 0.3 to 4.4% of recipients of cadaveric human GH, made and distributed before the mid-1980s, acquired Creutzfeldt-Jakob disease from it, depending on the country, with incubation periods up to 30 years (Brown et al., 2000). Current FDA labeling for one somatropin product lists indications in children with growth failure due to growth hormone deficiency or four other conditions, and in adults with growth hormone deficiency (FDA prescribing information for a somatropin product).

Choosing Between Them for a Research Question

Start with the level of the axis. Questions about GHRH receptor signaling, somatotroph responsiveness or endogenous pulsatile release under intact feedback call for a GHRH analog such as tesamorelin, whose effects depend on the pituitary (Dominikowski et al., 2026). Questions about the GH receptor itself or about actions downstream of the pituitary call for the hormone, which bypasses the pituitary and suppresses its output (Rosenthal et al., 1986; Wallace et al., 2001).

Measurement differs too. Recombinant GH has the same sequence as pituitary GH, and the randomized study above separated the GH group from the placebo group most clearly by pairing high 22 kDa GH with suppressed 20 kDa or non-22 kDa GH (Wallace et al., 2001). Tesamorelin is a distinct analyte: one study synthesized in vitro metabolites of tesamorelin and three other GHRH analogs as reference materials for an LC-MS/MS urine method (Memdouh et al., 2021).

Do not borrow findings across the pair. GH antagonizes insulin action in human subjects (Møller and Jørgensen, 2009), while two weeks of tesamorelin did not significantly change insulin-stimulated glucose uptake in 13 healthy men (Stanley et al., 2011); designs, populations and durations differ, so neither result predicts the other.

A PubMed search on 12 October 2026 for tesamorelin with somatropin, rhGH or recombinant growth hormone, limited to clinical trial publication types, returned 19 records; none randomized tesamorelin against GH. Plan arms in moles: one milligram holds about 0.19 µmol of tesamorelin and 0.045 µmol of GH. The Tesamorelin & Ipamorelin blend adds a ghrelin receptor agonist, so it cannot isolate the GHRH route.

Handling and Storage

Both tesamorelin listings ship lyophilized, and their product pages list -20°C for the powder as shipped and 2 to 8°C after reconstitution, protected from light, with freeze/thaw cycles avoided. Let the sealed vial reach room temperature before opening, add diluent slowly down the vial wall, and swirl rather than shake.

Tesamorelin can cloud or gel in near-neutral, unbuffered diluents and at high concentration (see our tesamorelin gelling note), and its methionine 27 oxidizes readily, so limit air, light and oxidizers. Aliquot single-use volumes, record the lot number and use the measured content from the COA portal, not the label mass.

Questions Researchers Ask

Tesamorelin vs HGH: what is the difference?

Tesamorelin is a 44-residue GHRH analog that acts on the pituitary, which then releases its own GH (Stanley et al., 2011). HGH is the 191-residue hormone, which acts directly on the GH receptor (de Vos et al., 1992).

Is tesamorelin a form of growth hormone?

No. It contains no GH; it is a releasing-hormone analog whose effects depend on the pituitary and stay shaped by somatostatin and IGF-1 feedback (Dominikowski et al., 2026).

Does tesamorelin or HGH change the pituitary's own GH output?

Yes, in opposite directions in the cited studies: tesamorelin raised overnight GH secretion in healthy men (Stanley et al., 2011), while exogenous GH blunted the GHRH response in normal men (Rosenthal et al., 1986) and suppressed endogenous isoforms in trained men (Wallace et al., 2001).

Does Disguised Alpha sell HGH?

No. Disguised Alpha does not sell growth hormone (somatropin). It sells tesamorelin and the Tesamorelin & Ipamorelin blend for laboratory research; HGH Frag 176-191 is a 16-residue fragment, not the hormone.

Are tesamorelin and HGH prohibited in sport?

Yes. The 2026 WADA Prohibited List names growth hormone, its analogues and fragments under S2.2.3 and GHRH and its analogues, including tesamorelin, under S2.2.4; class S2 is prohibited at all times.
